Forms and Insurance

  • New Patient forms include the Office Policy, Health History and Notice to Patients (HIPAA) Forms.
  • Download your new patient forms by clicking on them above or if you prefer, please ask the front desk to email them to you.
  • Please complete all forms and bring with you to your appointment along with your driver license (or other official form of photo ID) and insurance card. Please also bring any current laboratory results, x-ray, MRI, CT studies and reports.
  • We participate with a number of insurance companies. Please contact your insurance company to verify that Dr. Sutter is listed as a participating provider. If you have an HMO, Dr. Sutter is a specialist so a referral from your primary care physician will be needed. The office also provides several non-covered services, and payment for these is expected at the time of service. To clarify any concerns regarding your insurance, please contact our office.
  • If you must cancel your appointment please do so with 48 hours notice, as we reserve 45 minutes of the physician’s time for your new patient appointment. We know that your time is important so we do not double-book appointments so please show us the courtesy to cancel your appointment with 48 hours advance notice. We often have a waiting list, so someone else in need, could take advantage of the appointment time.

Interview & Exam

Dr. Sutter will conduct your interview in his office to review your health history, family history, current medications, current supplements, lifestyle assessment and the history of your current concern.  We set aside 45 minutes to ensure that your visit with Dr. Sutter and his Medical Assistant will be thorough and cover current and related areas of concern.

Dr. Sutter’s Medical Assistant will transfer you to an exam room and check your blood pressure.  Your physical examination may include among other things; range of motion, reflex and strength checks, muscle and joint exams, gait analysis, with examination of the affected area(s).  Dr. Sutter’s specialized knowledge of musculoskeletal conditions as well as his practice of lifestyle medicine for over 20 years will factor into your individualized interview and exam.

Testing & Biometrics

Dr. Sutter may recommend testing and biometric measurements such as blood work, BMI, body composition analysis,  condition specific questionnaires, diet logs,  x-ray, MRI, CT, and/or waist & hip measurements.

Lifestyle Medicine Treatment Plan

Your complete, personalized Lifestyle Medicine treatment plan will be based on the findings during your office visit as well as your test and biometric results.  It will include some or all of the following options.

  • Trigger Point, Tendon Sheath, Ligament, Muscle Attachment & other Pain Management Injections
